Welcome to WORK MATTERS Career Exploration Camp!

This Camp is for students between the ages of 14-22.

All participants must:

  • Complete this Registration Form and our Medical Release Form
  • Complete the Medical Care Authorization and Waiver and Release of Liability from WORK MATTERS.
  • If you are under the age of 18, the form must be completed by a parent or legal guardian.

Alcohol & Tobacco Policy

  • WORK MATTERS does not permit the presence or use of alcohol and/or tobacco products while attending the Career Exploration Camp.

Discipline Policy

  • We feel that all students deserve the opportunity to participate in a safe, healthy, and friendly atmosphere. With this in mind, WORK MATTERS staff have established rules that all of the students are expected to follow. Cussing, hitting, or verbal abuse directed at a student or a staff person may result in immediate suspension of your student from WM.
  • WM considers back-talking, arguing with WM staff, refusal to comply with program rules, and physical aggression toward students and staff to be unacceptable behavior.

Discipline Policy

  1. Rules will be clearly outlined. If inappropriate behavior occurs, redirection will be used to discourage the behavior.
  2. If behavior persists, a verbal warning will be given for inappropriate behavior.
  3. If behavior persists, we will contact you directly and request your student be picked up immediately. Students between the ages of 18-22 will be suspended from the program.
  4. Sexual misconduct of any kind will not be tolerated. Students will respect the personal boundaries of their follow students and all CEC staff. If sexual misconduct occurs, the student(s) will be suspended from the program.

Dress Code

  • WORK MATTERS follows the standard school dress codes and recommends closed-toe shoes and weather appropriate clothing.

Drop Off / Pick Up

  • You or an authorized sign-in/sign-out person must walk your student into the WORK MATTERS sign-in area and sign your student in with a WM staff member.
  • Students should be signed in by 8:15 AM.
  • Please call, text or email by 8:00 AM if your student will be absent from the WM Career Exploration Camp for the day. Thank you for your help.

  • Students must be signed out by an authorized pick-up person each day.
  • Students will not be released to a person that you have not designated as an authorized pick-up person unless WM receives a written parental request.
  • All WM participants must be signed out by an adult, 18 years old or older. This policy is based on legal counsel and is not negotiable.
  • WM will be happy to document as many adults as you need to authorize as pick-up persons. Please be prepared to show a photo ID for WM staff.
  • Students must be picked-up by 12:00 PM.

Medication Policy

  • Students who need to take medications during the WORK MATTERS Career Exploration Camp and related activities, must have their parent or legal guardian administer. This includes over the counter medications such as cough drops, sun screen, and medicated ointments.

Emergency Procedure

  • WORK MATTERS has developed an Emergency Plan. The Emergency Plan includes procedures for fire, illness, injury or other natural disaster.
  • WM also has staff members who are certified in CPR and First Aid.
  • In the event of a facility evacuation, WM will escort your student to the parking lot of Three Rivers Market, located at 1100 N. Central Avenue, at the Baxter Avenue intersection.
  • WM will inform you by phone and/or text message of an emergency. You will be told a brief description of the situation and you will be instructed as to where and when to pick up your student.
  • If your student is injured while at WORK MATTERS, you will be contacted immediately.
  • If your student requires immediate medical attention, he/she will be transported by ambulance to the nearest medical facility.
  • WORK MATTERS does not provide care for sick students. If your student has a temperature of 100 degrees or above, diarrhea, head lice, or any other contagious illness, do not send him/her to WM Career Exploration Camp. WM is concerned with the health and welfare of each student in the program. Please help us keep our students healthy by keeping sick students at home.
  • If your student becomes sick while at WM, a WM staff member will call you to pick up your student. Contagious students must be picked up within an hour of parental contact.
  • Please call WM by 8:00 AM if your student will not be attending WM Career Exploration Camp for the day.

  • WORK MATTERS tracks academic records and assessments for grant reporting purposes. WM also documents activities and events through pictures and videos. This tracking and documentation is essential to receiving continued grant funding.
  • Please be aware that an academic records and assessment release and a photo release statement are included on the Student Information Sheet. The records release is a requirement of program participation.

Zero Tolerance Policy

  • WORK MATTERS has a zero tolerance policy for weapons, dangerous objects, violence, or illegal drugs that will be aggressively enforced. Any student who is involved in a fight, in possession of a weapon, a dangerous object, or any controlled substance is subject to arrest and may face criminal proceedings. Violations will result in immediate suspension from the Career Exploration Camp.
